  •   Future Dermatology Clients: After scheduling an appt, please print out the Dermatology History Form located at the bottom of the Dermatology homepage, answer the 2-page questionnaire and fax to us (901-624-9014) no later than the week prior to the appointment. Please make sure to include BOTH your name as well as your pets’ name and put Attn: Kim on the fax
